Peptide Dose Tracker

Side Effect Log

You write down what you noticed, in your own words, and rate it on your own 1 to 5 scale if you like. Before your next visit, you can print your GLP-1 side effects journal as one list.

Your entries

Each entry holds the date, an optional time, what you noticed, an optional rating from 1 to 5 and a note.

You decide what the numbers mean. Nothing is scored, sorted by seriousness or flagged.

Saved entries appear newest first. You can delete one entry or all of them.

The print view shows a table with date, time, what you noticed, your rating and your note, oldest first. You can also export the list as a CSV file.

Before a visit, it helps to write down symptom details: how long they lasted, whether they changed and what happened after the visit1.

Free

The log on this page is free: entries, print, CSV export and delete. In the peptide tracker, notes on how you felt are free too.

Pro

In the tracker, Pro adds email and push reminders and a visit summary as a PDF. The price is shown before you pay.

What this log does not do

This log does not decide whether a symptom matters. It has no risk score and no check of your entries against a label table.

The Medication Guides say to talk to your healthcare provider about any side effect that bothers you or does not go away2. You can report side effects to FDA at 1-800-FDA-10883. In an emergency, call 911.

The Poison Help number is 1-800-222-12224. For agency notices in date order, see dated FDA alerts.
